All players registered with Lakefield Minor Hockey received an email today asking for a signed acknowledgement that will be uploaded to each players HCR profile.

Please make sure you review the concussion education material forwarded to you and return the signed form to Tanya before the start of the season.

Rowan's Law (Concussion Safety), 2018 places a legal responsibility on all Minor Hockey Associations to collect this information annually.
