ONLY Complete this form if you have a player who has never played hockey before. The Registrar will create a profile for them in HCR and email you when the profile has been created.

Players who will be 4 years old by December 31st must register here first. Once a profile is created by our Registrar, you will be required to complete the registration by going to: https://page.hockeycanada.ca/page/hc/ohf/ontario-minor-hockey-association/region-6-k/lakefield Please email a copy of your child's birth certificate to [email protected].
